This form helps UNTD faculty and staff communicate their space needs, whether that be a new space, a modification of current space or anything else related to a change in space at UNTD.

Facilities and Space Planning and Management review this form as an initial step. We will follow up with you for additional information. For help with this form contact SMP@untdallas.edu or 972-338-1855. Fields denoted with * are required.

Justification*Please describe why this project is needed to support your work, the mission of your department and of UNTD. Include the goals/objectives of the requested work. This may not apply to simpler requests, such as painting an office, so you may note that this is not applicable.
