  Financial aid forms are documents students must complete to apply for and maintain eligibility for federal, state, and institutional financial aid. UNT at Dallas may also require additional forms—such as verification documents, income worksheets, or appeals—to confirm student information or review special circumstances. Completing all required forms accurately and on time helps ensure financial aid can be processed without delays.

Most Common Financial Aid Forms

Financial Aid Verification Forms – Used when a student is selected for verification; may include household size, income, or tax information worksheets.

SAP Appeal Form (Satisfactory Academic Progress Appeal) – For students who do not meet SAP standards and wish to appeal for aid reinstatement.

Dependency Override Request – For students requesting independent status due to unusual circumstances.

Special Circumstance/Professional Judgment Form – Allows students to report significant changes in financial circumstances.

Counselor Action Request Form – For students who want to accept, increase, reduce, or cancel student loans.
