Financial Aid Verification Forms – Used when a student is selected for verification; may include household size, income,
or tax information worksheets.
SAP Appeal Form (Satisfactory Academic Progress Appeal) – For students who do not meet SAP standards and wish to appeal for aid reinstatement.
Dependency Override Request – For students requesting independent status due to unusual circumstances.
Special Circumstance/Professional Judgment Form – Allows students to report significant changes in financial circumstances.
Counselor Action Request Form – For students who want to accept, increase, reduce, or cancel student loans.
Financial aid forms can be signed using a pen or by adding an Adobe e-signature as long as the form is submitted through your student email account. Below are steps on how to set up an e-signature:
